The problem doesn't seem to be my mic, it's somewhere in the Voice FX, and I'm not sure what I haven't set up properly.
I'm using SAM 4.2.2, with a Logitech USB headset.
I understand "USB Headset" raises a red flag with SAM, but it works fine... once.
The issue is, I can use the Voice FX normally, using either the Push-to-talk or lock feature just fine. However, once I've clicked the "Mic Off" button, my voice will not then record again using the "Mic On" button. If I choose this option a second time-- a few seconds to a few minutes or an hour later, doesn't matter-- the music will fade like it's supposed to, but I cannot record my voice.
It appears to have something to do with the EQ preset I've chosen, which is custom. However, I've also tried deleting my custom preset and using the default, and I have the same problem.
My customer preset (I have a soft voice, the default barely registers my voice):

Only one of the other DJ's has had this problem, and he fixed his by using the config dialog box, but it doesn't work for me.
My config dialog:
